• All required intake forms, consents, and documentation must be completed in full prior to appointment confirmation.• Failure to complete required documentation may result in appointment cancellation, delay, or rescheduling at the practitioner’s discretion.• Annual Assessment Requirement: A comprehensive assessment is required prior to initiating services and must be completed annually to maintain active client status and continue receiving services.
• Appointments are scheduled no more than 14 calendar days in advance unless otherwise specified.• A 15-minute grace period is provided. If arrival exceeds this window, a late fee may be applied or the appointment may be rescheduled.• Missed, late, or cancelled appointments outside the stated policy window are non-refundable.• All booked appointments are non-refundable and non-transferable.
• In-home services include professional travel time, preparation, and scheduling buffers to ensure service quality, safety, and continuity of care.
• Phlebotomy services involve blood collection only and are not considered injection therapy.• Blood samples are collected by the practitioner and submitted to independent, CLIA-certified laboratories for processing and analysis.• Laboratory processing timelines, reference ranges, and reporting standards are determined solely by the laboratory.• Laboratory results are reviewed and shared for educational and wellness guidance purposes only.• Laboratory Requirements for Services: A Comprehensive Metabolic Panel (CMP) and Complete Blood Count (CBC) completed within 30 days are required for all new clients. Results outside this timeframe will not be accepted, and updated testing will be required prior to initiating services. Clients may complete laboratory testing through The Origin Method or provide qualifying external results at the practitioner’s discretion.
• Supplements and nutraceuticals are purchased separately unless otherwise specified.• Recommended supplement brands are not manufactured or sold by The Origin Method. Clients may choose their own brands and follow practitioner-guided wellness recommendations.• Clients are responsible for reviewing supplement labels and disclosing all medications, supplements, and allergies.• No guarantees are made regarding supplement effectiveness or outcomes.
• The Origin Method does not perform injection therapies or IV administrations. Any IV formulation or protocol guidance provided is strictly for educational and wellness purposes and does not constitute medical orders, prescriptions, or treatment authorizations.• IV and injection administrations are performed by independent, licensed medical providers who are solely responsible for their own intake, screening, informed consent, clinical decisions, administration, and outcomes.
